Health Choice believes in a personal approach to health care right in your community. We built our health care plan around you. Our goal is to give you quality health care, programs, and services to support you on your path to wellness.

Health Choice Utah offers care in all 29 counties across the state.

As a member, you and your family will have access to:

  • A wide range of doctors, pharmacies, hospitals, and specialists
  • Care in your language
  • Doctors to help you and your family get quality health care close to home
  • A Nurse Advice Line with 24/7 access to a registered nurse
  • Our Stork Program where we help you during and after your pregnancy

If you would like to change your health plan, call your state HPR at 866-608-9422.
