When submitting claims with Place of Service (POS) 99 – Other Place of Service, it is essential to clearly document where the services were actually rendered. POS 99 is used only when the service location does not meet the definition of any other established POS code. Because this code is so broad, additional detail is required to ensure accurate claims processing, proper compliance, and correct provider directory and audit validation.
WHAT PROVIDERS MUST INCLUDE
If billing POS 99 on a claim:
- CMS-1500 (paper claims)
List the name and physical address of the service location in Box 32 – Service Facility Location Information. - Electronic claims (837P)
Populate Loop 2310C – Service Facility Location with the name and physical address where the service occurred.
This information must reflect the exact site where the member received services—such as a community setting, daycare, park, restaurant, etc., or other non‑traditional environments.
WHY THIS MATTERS
Providing the complete service location helps ensure:
