Prior Authorization List: Removing Non-Covered Codes for Greater Clarity 

Health Choice Utah requires prior authorization for all elective inpatient stays as well as certain outpatient services. To better support our members in accessing the care they need, we are working to make it easier for providers to understand which outpatient services are covered and which require prior authorization.

What’s Changing 

To improve the usefulness and clarity of our prior authorization list, we will begin removing any codes that are specifically excluded from a plan’s benefits. Our goal is to make the prior authorization list as transparent and efficient as possible. Removing non-covered codes will help reduce unnecessary confusion and prevent avoidable denials.

Please note that while we strive for accuracy, individual plan benefits or unique member circumstances may result in exceptions. We encourage providers to verify coverage details when needed.
