Billing Reminder: One CMS‑1500 per Place of Service (POS)

To ensure accurate and timely claims processing, we want to remind all providers of the correct billing requirements when patients are seen in multiple locations.

When a patient receives care in more than one setting—such as home, office, or telehealth (not originating from the home)—providers must submit separate CMS‑1500 claim forms for each distinct place of service (POS).

Submitting multiple POS codes on a single claim will result in the claim being denied.

WHAT THIS MEANS FOR YOUR PRACTICE

  • Submit one CMS‑1500 claim per place of service.
  • Do not combine different POS codes on the same claim.
  • Claims that include more than one POS will be denied and require resubmission, which may delay payment.
  • Claims will be denied with the following codes:
    • CO16 – Lacks information needed for adjudication
    • M77 – Missing, incomplete, or invalid POS
