How to Find Your Network Participation and Navigate the Provider Directory
Step 1: Visit the Main Website (Public Search)
- Go to healthchoiceutah.com.
- Click "Find a Doctor" in the upper right-hand corner.
- Select a provider network before starting your search.
Step 2: Use Filters to Narrow Your Search
To find clinic network participation:
- Select "Clinics" under filters, scroll down, and click "Search Providers."
- Use the Medical Group field to search for your practice or clinic name.
- Scroll down and click "Search" again.
- Locate your clinic in the results and double-click the practice name.
- In the additional information section, you will see the networks your clinic is contracted with.
To Find Facility-Based Provider Networks
- Select "Other Providers" under filters, scroll down, and click "Search Providers."
This includes Home Health Agencies, Durable Medical Equipment providers, and Residential Treatment Facilities. - Enter the facility name to refine your search.
- Scroll down and click "Search" again.
- Locate your facility in the results and double-click the name.
- In the additional information section, you will see the networks your facility is contracted with.
